- How it works
- Sofosbuvir is a prodrug metabolized intracellularly to its active uridine triphosphate analog. This active metabolite is incorporated into the nascent HCV RNA chain by the viral NS5B RNA-dependent RNA polymerase, acting as a non-obligate chain terminator due to modifications that block continued chain elongation. Because it targets a viral enzyme essential for replication rather than a human cellular process, it selectively disrupts HCV genome synthesis. It is always used as part of a combination regimen to maximize sustained virologic response and limit resistance emergence.
- Therapeutic applications
- FDA-indicated for chronic hepatitis C virus infection across genotypes 1-4, in combination with ribavirin and, for some regimens, peginterferon alfa; in patients with hepatocellular carcinoma awaiting liver transplantation; and in pediatric patients 3 years and older with genotype 2 or 3 infection.
