- How it works
- Lisinopril inhibits angiotensin-converting enzyme (ACE), preventing the conversion of angiotensin I to angiotensin II, a potent vasoconstrictor and stimulus for aldosterone secretion. Suppression of the renin-angiotensin-aldosterone system results in decreased plasma angiotensin II, reduced vasopressor activity, and decreased aldosterone secretion, lowering peripheral vascular resistance and blood pressure. Because ACE also degrades bradykinin, a vasodilator peptide, its inhibition may increase bradykinin levels, contributing additional vasodilatory effect. In heart failure and post-MI settings, this reduction in vasoconstriction and afterload decreases cardiac workload and is associated with reduced mortality.
- Therapeutic applications
- Indicated for the treatment of hypertension in adult and pediatric patients 6 years of age and older; as adjunct therapy in the management of systolic heart failure; and for reduction of mortality in hemodynamically stable patients within 24 hours of acute myocardial infarction.
