- A lab-made antibody that latches onto IL-23, a signaling protein that drives inflammation, blocking it before it can trigger the immune overactivity behind psoriasis, psoriatic arthritis, and inflammatory bowel disease.
- How it works
- Guselkumab is a human IgG1λ monoclonal antibody that selectively binds the p19 subunit of interleukin-23, preventing it from engaging its cell-surface receptor. Because IL-23 sits upstream of Th17-driven inflammatory signaling, this blockade suppresses downstream release of proinflammatory cytokines implicated in psoriatic and inflammatory bowel disease pathology. Unlike agents targeting the shared p40 subunit, this p19-selective binding spares IL-12 signaling.
- Therapeutic applications
- FDA-indicated for moderate-to-severe plaque psoriasis in adults and pediatric patients 6 and older weighing at least 40 kg, active psoriatic arthritis in the same population, moderately to severely active ulcerative colitis, and moderately to severely active Crohn's disease in adults.
