How it works
Montelukast is a selective, orally active leukotriene receptor antagonist that binds with high affinity to the cysteinyl leukotriene CysLT1 receptor. By inhibiting CysLT1, it blocks the physiologic effects of leukotriene D4 and related cysteinyl leukotrienes, potent inflammatory mediators released from mast cells and eosinophils during allergic and asthmatic responses. This blockade reduces bronchoconstriction, airway edema, mucus secretion, and eosinophilic inflammation. Unlike bronchodilators, it targets the inflammatory cascade upstream rather than providing acute smooth-muscle relaxation, making it suited for chronic prophylactic use.
Therapeutic applications
Indicated for prophylaxis and chronic treatment of asthma in patients 15 years and older; acute prevention of exercise-induced bronchoconstriction in patients 15 years and older; and relief of symptoms of seasonal and perennial allergic rhinitis in patients 15 years and older.
