How it works
Filgrastim is a recombinant form of human granulocyte colony-stimulating factor (G-CSF) that acts on specific cell surface receptors on hematopoietic cells to regulate neutrophil production in the bone marrow. It stimulates the proliferation, differentiation, and functional activation of neutrophil progenitors — including enhanced phagocytic ability and increased cell surface expression on mature neutrophils — with the FDA label noting minimal direct effect on other hematopoietic cell lineages. This lineage-selective action raises circulating neutrophil counts without broadly stimulating the rest of the blood cell-producing system.
Therapeutic applications
Indicated to decrease the incidence of infection, as manifested by febrile neutropenia, in patients with nonmyeloid malignancies receiving myelosuppressive anti-cancer drugs; to reduce time to neutrophil recovery and duration of fever following induction or consolidation chemotherapy in acute myeloid leukemia; to reduce the duration of neutropenia and related complications in patients undergoing myeloablative chemotherapy followed by bone marrow transplant; to mobilize hematopoietic progenitor cells into peripheral blood for collection by leukapheresis; for chronic administration to reduce infection incidence in patients with severe chronic neutropenia; and to increase survival in patients acutely exposed to myelosuppressive doses of radiation.
